Hyperbaric Chamber Therapy for Brain Injury and Stroke. We evaluated the effect of hyperbaric oxygen (HBO) therapy on neurological function and infarct size in 33 unanesthetized cats subjected to temporary 6-hour or 24-hour occlusion of the middle cerebral artery (MCA) 7 to 10 days after transorbital implantation of a vessel occluder. HBOT therapy (100% oxygen at 1.5 atmospheres absolute) was administered for 40 minutes during or after 6-hour occlusions and before, during, and after 24-hour occlusions. Neurological function was graded on a scale of 0 to 10 every 30 minutes before, during, and after occlusion and HBO treatments until it stabilized and then daily until the cats were killed 10 days after occlusion.

The results were compared with observations in 13 untreated controls and 6 cats that received 100% O2 at atmospheric pressure during a 6-hour MCA occlusion. HBO therapy during the 1st or 3rd hour of a 6-hour MCA occlusion resulted in a four-grade improvement of the initial neurological function; this effect persisted during the remainder of the occlusion. The average grade of neurological deficit at death was 94% less than in the untreated cats (P < 0.03).

Infarct size in the HBO-treated group was 58% less than in controls (P < 0.03). There was no significant difference in infarct size between the untreated cats and those treated with 100% O2 at atmospheric pressure. HBO therapy during the 4th hour of a 6-hour MCA occlusion had no statistically significant effect on infarct size, even though the mean neurological deficit was 73% less than in controls (P < 0.02).

Cats treated with HBO during the 3rd hour of a 24-hour MCA occlusion had a four-grade (74%) improvement in neurological function (p < 0.02) and a 41% reduction in infarct size (P < 0.001) compared with the untreated controls. HBO therapy after 6-hour and 24-hour MCA occlusions was of no benefit. These results demonstrate that HBO therapy administered early after MCA occlusion had a significant protective effect during temporary regional ischemia in cats. (Neurosurgery 20:518-524, 1987)

Introduction

Dr. Gunnar Heuser, a toxic exposure specialist, documented results of a series of hyperbaric treatments on 9 patients suffering from Impaired Brain Function. The purpose was to study the effects of 1.3 ATA of hyperbaric therapy on those patients. His patients included individuals who suffered from impairment to brain function such as memory and balance due to years of exposure to a variety of chemicals, pesticides and solvents. Some patients had developed attention deficit disorder and may well have been experiencing disabling effects that altered their quality of life, It is important to note that MRI scans confirmed that there was no cell death in those patients, only loss of function.

SPECT imaging

SPECT stands for Single Photon Emission Computed Tomography. A scan is performed by injecting the patient with a radioactive substance that travels through the brain and distributes itself according to the circulatory system. The patient’s brain is then scanned and a computer creates a three dimensional color image that illustrates the blood flow into all areas of the brain. SPECT i maging helped us by identifying the parts of the brain that are dormant due to a lack of oxygen. A normal, healthy adult brain should read primarily yellow on a SPECT scan. Green is indicative of less blood flow and oxygen. Blue and violet represent significantly reduced blood flow in the brain.

Methods

Each patient was given a series of 10 hyperbaric treatments: one hour per day for five consecutive
days for two weeks. To objectively test the effect of the hyperbaric sessions, SPECT scans were taken of
each patient’s brain before and after the series of treatments.

Results

Every one of the patients who participated in the study showed improvement after only 10 treatments. Reporting fewer problems with short term memory, balance, headaches, and other symptoms of poor circulation in the brain. The SPECT scans objectively confirmed an increase in blood flow and oxygen to effected areas of the brain such as the temporal lobe, the part of the brain where short term memory is located. The improvements in the temporal lobe seen in the SPECT scans were verified when the patient regained his short term memory.

Discussion

The benefits of treating toxic exposure patients with Hyperbaric Therapy appear to be very positive.
Initial results indicate that Hyperbaric Therapy has a demonstrable and positive effect on some symptoms such as short term memory loss and balance. This study and additional research will add to our understanding of the exciting possibilities of Hyperbaric Therapy.

A 5-year study of patients with severe traumatic brain injury conducted at Hennepin County Medical Center in Minneapolis shows significant benefit of hyperbaric oxygen therapy to improve brain metabolism and its ability to recover from injury. The results were recently published in the Journal of Neurosurgery.

Every year, more than 1.4 million Americans sustain a traumatic brain injury (TBI) – the leading cause of death and disability in children and young adults. Those who survive often face months or even years of therapy, and sometimes the damage to the brain is irreversible. Decreased utilization of oxygenated blood to brain tissue immediately after the injury is often to blame.

Cells need oxygen to fuel metabolism for cellular growth and repair. Healthy brains produce their own energy to maintain brain tissue and keep the rest of the body doing what it’s supposed to do. That includes automatic processes like breathing and circulation, as well as voluntary actions like walking and talking.

After a traumatic brain injury, the brain itself needs care. Barriers to blood flow can be compromised from the impact of the injury itself, and then when the brain swells inside the skull, a secondary injury can occur that causes even more brain damage.

“There’s a direct correlation between clinical outcome and the degree to which the brain’s metabolism is restored,” explains one of the study’s authors, neurosurgeon Gaylan Rockswold, MD. “In previous research we learned that the brain’s energy production is improved and maintained with hyperbaric oxygen treatment, but this study confirms that hyperbaric oxygen treatment has a major impact in terms of increased energy production.”

Within 24 hours after injury, eligible patients for the study were randomized into three groups: One group received “normobaric” treatment: oxygen delivered at the patient’s bedside; another group received hyperbaric treatment in Hennepin County Medical Center’s hyperbaric oxygen chamber; and a third (control) group did not receive additional oxygen therapy. All groups received the intensive standard of care for brain injury consistent with good clinical practice. The patients who received higher levels of oxygen (hyperoxia) via the hyperbaric oxygen chamber were found to have a marked increase in positive brain metabolism when compared to the normobaric and control group.

“Our goal was to evaluate the brain’s metabolism and intracranial pressure, and whether or not too much oxygen posed a concern with hyperbaric oxygen treatment in these patients,” said Dr. Rockswold. “The results indicate that hyperbaric oxygen treatment was found to significantly enhance the brain’s energy production and reduce intracranial pressure without any toxic effects on the brain or lungs from too much oxygen.”

This research provides important preliminary data for a National Institutes of Health (NIH) supported multicenter trial. NIH trials directly assess the ability to improve clinical outcomes, which is the final step needed to change standard clinical processes. Currently standard clinical practice does not include hyperbaric oxygen for traumatic brain injury.

“TBI is not only devastating for the patient, it’s also heart wrenching for his or her family. We couldn’t be more pleased about the impact this study will have for patients with traumatic brain injury.”
