The prostate is a small organ about the size of a walnut. It is found below the bladder (where urine is stored) and surrounds the tube that carries urine away from the bladder (urethra). The prostate makes a fluid that becomes part of semen. Semen is the white fluid that contains sperm.

These medications must be taken indefinitely or the prostate will start to grow again. Complications are minimal with the rare reported case of decrease in desire or loss of sexual ability.

An interesting side effect of the 5-alpha reductase inhibitors is reversal of baldness, a nice benefit for bald guys. On the other hand, few of the men I treat mention regrowth of hair.

Both Proscar and Avodart reduce PSA (prostate-specific antigen, measured for the blood test to detect early prostate cancer) by about half. After a period of a year, one can continue to monitor and screen for prostate cancer with this test, but it must be remembered that the values will be reduced. At the same time, Proscar has been shown to reduce the risk of prostate cancer by about 25%. Studies for similar effects by Avodart are ongoing, but I think it will also reduce risk.

Alternatively, alpha blockers, a class of medications sometimes used to treat high blood pressure, have been found to relax the muscle fibers within the prostate and outflow tube. This relaxation enlarges the channel and decreases the blockage. The medications do not shrink the prostate, and the prostate will continue to enlarge in size. They are, however, quite effective at reducing symptoms.

Flowmax is the most popular of the alpha blockers, followed by Uroxatrol. They have less tendency to reduce blood pressure than the old-line (and now generic) medications Hytrin and Cardura, but none of these medications is likely to kill two birds with one stone.
The symptoms will return immediately if the medication is stopped. Studies have shown a success rate of 40-50% in terms of a decrease in symptoms and improving urinary flow. Side effects include dizziness, fatigue, light headedness and fainting. For the first few days, the medication should be taken with caution at night, since when getting up to urinate there is the potential to pass out.

Nothing says that these two classes of medications can’t be used together, and that is, in fact, a common approach. A typical regimen after evaluation might be to start Flowmax and return in two weeks for further evaluation. If prostate cancer has been ruled out, PSA is over 2, and a digital rectal exam indicates enlargement, then I add Avodart. About a third of men can stop the Flowmax after three months, 60% can drop it at six months, and 90% are off within nine months. As noted previously, Avodart can also be reduced over time. especially considering the fact that most products work so poorly and help so little.

The first cause is hormonal. After about age 40 more of the 5-alpha-reductase enzyme is produced by men. This enzyme converts testosterone, which we all want to have more of as we age, to dihydrotesterone or DHT. As men age, DHT levels tend to raise because of this increased enzyme production. DHT causes the prostate to increase in size. Higher DHT levels are one of the major causes of enlarged prostates. However they are not the only cause. If they were, herbal formulas containing saw palmetto would work better than they do.

It seems that the majority of men suffering from enlarged prostates are not helped significantly by herbal formulas containing saw palmetto, pumpkin seeds and other herbs which block the formation of DHT and may inhibit its binding to cell receptors in the prostate. Most likely this is for two reasons. First is that these herbs don’t work as powerfully as their promoters would like us to believe. Secondly, they don’t deal with what may be an even more major cause of an enlarged prostate. Infections.

Our guess is that approximately 2/3 of all men suffering from an enlarged prostate have an infection in the prostate. This infection could be viral, bacterial, fungal or even mycoplasm. Infections tend to cause inflammation. An inflammation in the prostate enlarges it. And of course you may suffer from both an infection and increased DHT levels. Your doctor may be able to give you an antibiotic to help you with a bacterial prostate infection, but he has little to help with viral and mycoplasm infections.

For years this report has recommended unique products for improving the symptoms of an enlarged prostate. These products worked better than the usual saw palmetto and sterolin based herbal formulas.

Because they helped many people that were not helped by these usual types of herbal formulas. The first product was U-Fn, a u-fucoidan extract from brown algae that was expensive but better than the herbs. Unfortunately, many people still were not helped. So a few years later we heard about an oxygenating, detoxifying, pH balancing liquid supplement that fights infections. Zeo 3. For most patients suffering from an enlarged prostate who do not have kidney damage or serious infection, the ultimate decision must be left to the patient. While there are absolute indications of intervention, most situations depend on how bothersome the symptoms are to the individual. Sometimes men may take antibiotics for a long period of time without any true benefit. What is advanced prostate cancer?

Can advanced prostate cancer be cured?
Currently, no cure exists for advanced prostate cancer. Men can benefit, however, in several ways. Some treatments slow the growth of the cancer. Others reduce symptoms caused by the disease. Until recently, no therapy was offered to prolong life. However, the recently approved chemotherapy drug Taxotere, when taken with the steroid prednisone, has been proven to extend survival by several months. Taxotere is approved only for men with cancer that no longer responds to hormone therapy (called hormone refractory prostate cancer).

In addition, Provenge, a “”vaccine”" for the treatment of advanced prostate cancer, does appear to extend survival — by months for most patients, by years for some. Provenge isn’t your everyday vaccine. It’s an immune therapy created by harvesting immune cells from a patient, genetically engineering them to fight prostate cancer, and then infusing them back into the patient. It’s approved only for treatment of patients with few or no symptoms who have prostate cancer that has spread outside the prostate and no longer responds to hormone therapy.

Radiation Therapy – Called Radical Radiotherapy this procedure is used when the cancer is localized and has not spread. Radioactive isotopes are shot into the area where the cancer is located by external radiotherapy- (short bursts of high energy beams directed by x-rays). These bursts are shot into the area where the cancer cells are. The treatments are performed on a daily basis. Your doctor will tell you how long these treatments will last. Radiation therapy does not always cure the patient of cancer, especially if the cancer is more widespread than thought. This treatment does have side effects, so it is important you talk to your doctor beforehand he will prepare you for what to expect.
An alternative used of radiation uses cobalt irradiation (gamma rays) to attack the cancer cells. There are side effects to this procedure as well, including the dangers or anemia and chronic fatigue.

Wait and See – If the doctor tells you that he wishes to wait and just monitor you every 3-6 months, than your prostate cancer is really only in the clinical stages and not far-advanced. You will be closely watched during that time frame. Sometimes, no treatment is warranted.

Although the word cancer is scary and prostate cancer is so prevalent. Being diagnosed with prostate cancer does not mean you are being handed a death sentence. With today’s aggressive prostate cancer treatments and the research catching up to the medicine, many men who have been diagnosed with this disease are living healthy and happy lives after treatment. Remember to talk to your doctor. Get several different medical opinions and join a support group to help you through the turbulent times ahead.
Among men under 65 with early stage prostate cancer, those who have the prostate gland removed are less likely to die than those who adopt a “”watchful waiting”" approach, according to a new long-term study out of Europe.

But the findings are not likely to be practice-changing, said Dr. Richard E. Greenberg, chief of urologic oncology at Fox Chase Cancer Center in Philadelphia, who was not involved with the study.
“”This is relatively early stage disease, and it shows what we would expect it to show: that those who have more years [to live] have better survival if treated aggressively,”" he said.

Men with prostate cancer face a confusing maze of options today, including not only surgery but hormone therapy, different kinds of radiation therapy and even simply foregoing medical treatment while monitoring the cancer closely (“”watchful waiting,”" also known as “”active surveillance”").
“”Watchful waiting”" is often recommended when a man is not expected to die of the cancer and would like to avoid the risk of the debilitating side effects associated with prostate cancer treatment. These may include: incontinence and erectile dysfunction (surgery); erectile dysfunction and secondary cancers (radiation therapy); nausea, breast growth, liver problems (hormonal therapy); and weakness, hair loss, fluid retention (chemotherapy).

The data presented in the May 5 issue of the New England Journal of Medicine is a 15-year follow-up of a study for which three-years of follow-up had previously been reported.
The earlier study also found that undergoing a radical prostatectomy lowered the risk of the cancer spreading, as well as the death rate from prostate cancer or any other cause.

The researchers, Drs. Anna Bill-Axelson and Lars Holmberg of University Hospital, Uppsala, and colleagues, enrolled nearly 700 men with early prostate cancer under the age of 75 who were randomly selected either to undergo a radical prostatectomy or to stay in “”watchful waiting”" mode.
After an average follow-up of about 13 years, 14.6 percent of the men who had undergone surgery had died, versus 20.7 percent in the watchful waiting group, a 38 percent reduced risk.

The benefit was primarily seen among men younger than 65 and the authors estimated that seven men under the age of 65 and 8 over the age of 65 would need to be treated to save one life.
There was also a benefit in men whose tumors had low-risk characteristics.
Men whose cancer had moved outside of the prostate gland and who underwent removal of the prostate had a seven times higher risk of dying than men whose tumors hadn’t spread.

Radical prostatectomy can come with a price — including erectile dysfunction, urinary incontinence and even bowel incontinence — although these effects are not as common as techniques improve. A nerve-sparing radical prostatectomy, for example, may be able to prevent erectile dysfunction.
Also, the men in this study were diagnosed only after they had symptoms, unlike the majority of men in the United States, Greenberg said.

“The advantage of screening is that men are more likely to be cured of cancer but they also have a greater chance of not succumbing to the consequences of the surgical complications, whether [those] be erectile dysfunction or urinary incontinence,”" Greenberg said.
Also, he said, erectile dysfunction can also be a factor of aging and can be treated in most cases.
But much remains to be known about how best to treat different types of prostate cancer.

“The management of early stage prostate cancer continues to require further randomized controlled studies, which are underway, to determine the true benefit of treatment versus expectant management,”" said Dr. Marc Danziger, an attending urologist at Lenox Hill Hospital in New York City, who was not involved with the study.
“”Currently, the potential benefit and side effects of treatment needs to be addressed on an individual basis in relation to the risk stratification of disease, patient desires, health status and risk tolerance,”" Danziger added.
