Hyperbaric oxygen (HBO) therapy is a medical treatment that uses the administration of 100 percent oxygen at a controlled pressure (greater than sea level) for a prescribed amount of time—usually 60 to 90 minutes.

The air we normally breathe contains 21 percent oxygen, 78 percent nitrogen, and 1 percent carbon dioxide and all other gases. Increasing the pressure of the air we breathe through the administration of HBO therapy has the effect of dramatically increasing the number of oxygen molecules entering the bloodstream, up to 2000 percent. This increase in oxygen under controlled pressure means that oxygen can reach bone and tissue that is normally not accessible to red blood cells. Red blood cells carry oxygen throughout the body.

Hyperbaric Oxygen Therapy (HBOT) is a specialized medical intervention in which the atmospheric pressure is increased above 1 ATA (above sea level-14.7 psig), usually by the filling of a single person chamber, multi-place chamber or a hyperbaric room with a dose specific amount of oxygen (atmospheric oxygen or enriched oxygen). At sea level, our lungs absorb a certain amount of oxygen molecules from the air. When descending to lower altitudes (below sea level), the pressure is greater (above 1ATA) and now the lungs more easily absorb the compressed oxygen molecules in the air.

In contrast, at higher altitudes, when the pressure drops, the lungs work harder to absorb the dispersed oxygen molecules from the air. This is why oxygen masks deploy in an airplane during sudden high altitudes changes, helping to increase the O2 content due to a lack of pressure.

During a hyperbaric “dive” the fluids and tissues of the body receive an infusion of readily available oxygen. In fact, even cells and areas of the body with limited circulation become saturated in oxygen. The effect is an uptake of oxygen in the blood, plasma cerebral-spinal fluids, and tissues. In addition, the vaso-constrictive nature of hyperbaric therapy has an added effect of reducing inflammation and edema.
Therapy Options

There are many different manufacturers and designs of HBO chambers–monoplace, multiplace, and portable. The treating physician’s protocol will dictate which type of chamber should be used.
Uses of HBOT

Currently, there is an increase in the amount of research being conducted on hyperbaric medicine in modern practice. Published studies in leading journals, such as The New England Journal of Medicine document the use of HBOT for the treatment of such indications as multiple sclerosis, acute carbon monoxide poisoning, gas gangrene, air embolism, diabetic foot wounds, infections, burns, ulcers and edema. But these extablished indications only begin to open the current chapter in the use of modern hyperbaric technology.

Hyperbaric Oxygen Therapy (HBOT) is the administration of 100 percent oxygen via inhalation in a pressurized chamber. HBOT is an adjunctive therapy used with conventional and primary therapies to enhance treatment and recovery. HBOT is approved for use in humans and animals.

The Procedure

1. The pressure inside the hyperbaric chamber is measured in atmospheres and is usually between 1.5 and 2.4 atmospheres. That is roughly the equivalent of 16 to 46 feet below sea level. The patient remains in the chamber, breathing in pure oxygen, for however long the condition being treated requires. With cats, dogs and horses, more nervous animals will be sedated first and usually sleep through the procedure. Someone monitors continuously while the animal is in the chamber.

History

2. The use of hyperbaric therapy dates back to 1662, when a British clergyman invented the domicilium to expose patients to higher pressures for speeding healing. His ideas weren’t based on science, and the use faded from medical practices. In the 1800s, his theory enjoyed a resurgence in popularity, and pneumatic institutes opened all across Europe, usually in conjunction with health spas. In 1879, Dr. Fontaine, a French surgeon, began performing operations in a mobile hyperbaric chamber of his own design. He noted patients were more deeply sedated in the chamber, and incisions healed faster. True HBOT started about 1955, with the introduction of pure oxygen, instead of compressed air, into the chamber.

Therapeutic Effects

3. HBOT has a number of therapeutic effects. It can stimulate cellular growth, the formation of capillaries to improve blood flow, speed up the immune system’s elimination of microbes, interrupt the reproduction of bacteria and viruses and reduce swelling. HBOT is also effective for countering hypoxia (lack of oxygen) following an accident, such as drowning or carbon monoxide poisoning.

Conditions
4. Several conditions respond to HBOT including brain injuries, spinal cord trauma, stroke, cardiac disease, severe sinusitis, seizures of unknown origins, abdominal sepsis, pancreatitis, tissue necrosis and even gingivitis.

Side Effects

5. Hyperbaric oxygen therapy has few side effects, but toxicity can occur if the animal receives too many atmospheres of pressure for too long. Signs of toxicity include convulsions, muscle twitching, dizziness, anxiety and nausea. Other side effects include pneumonia from too much air introduced into the lungs. Occasionally, animals can experience coma or respiratory failure.

The Slavica BioChem division of Hard to Treat Diseases, Inc. (HTDS) recently presented results from their hyperbaric oxygen (HBO) chamber study for treatment of traumatic brain injury. The positive results were presented to the 8th World Congress on the Brain Injury in Washington, DC in March, put on by the International Brain Injury Association. The gathering is the largest of its kind in the world. This year’s meeting was the most heavily attended ever, featuring therapists, social workers, medical professionals, psychologists, and researchers from all over the world, a PR Newswire article reported.

The head scientist of HTDS, Dr. Sanja Pekovic, said in the article, ‘This Congress was an excellent opportunity to get insight into the state-of-the-art research, covering all from the basic science to the clinical aspects of brain injury, as well as for establishing important contacts with international professionals involved in the brain injury research and the care of people with acquired brain injury.’He added that the conference was a perfect venue to present the results of the Serbian team’s hyperbaric oxygen chamber animal trials for brain injury treatment.

While rats were used in the study, the same hyperbaric oxygen chambers used for humans were used in the experiment. Slavica Biochem conducted the study in conjunction with researchers from the Hyperbaric Medical Center at the School of Medicine at University of Belgrade in Serbia. The results of the study were promising and added new data upon their previous positive achievements.

Dr. Pekovic continued, ‘Our previous experiments showed that repetitive HBO treatments significantly reduced neuronal loss and degeneration, suggesting that HBO was able to attenuate the effects of brain damage by reducing the progression of neuronal injury, the article reported. He added that his team’s most recent study ‘revealed new beneficial effects of HBO treatment on glial scarring and inflammatory processes after traumatic brain injury. The positive results did not end there.

Pekovic asserted that HBO treatments reduced inflammation in the brains of the rats in the study and suggested that HBO be used for brain injury recovery as soon as it becomes available for such use. Pekovic ended by calling HBO chambers an ‘attractive therapeutic tool for improving recovery from head injury.’ With any luck, the treatments will become widely available sooner than later.

Traumatic Brain Injury Treatment

A team with the San Antonio Military Medical Center Hyperbaric Center and the Air Force School of Aerospace Medicine received funding to study the effects of hyperbaric oxygen therapy on mild to moderate traumatic brain injuries.

Researchers hope to find additional ways to treat wounded warriors with traumatic brain injuries using the hyperbaric center, located at Wilford Hall Medical Center, or SAMMC-South, here.

Traumatic brain injury is common with head injuries caused by blows to the head, nearby explosions, concussion or penetrating wounds. These types of injuries have become relatively common in U.S. military forces who have served in Iraq and Afghanistan. Treatment of traumatic brain injury normally relies on traditional rehabilitative and retraining strategies or on the use of drugs to reduce symptoms, such as depression and anxiety.

The Air Force study will try to determine if hyperbaric oxygen therapy improves the cognitive function of individuals who have had traumatic brain injury. Cognitive function includes such things as thinking, remembering, recognition, concentration ability and perception.

Oxygen is a vital component in the body’s healing process. Hyperbaric oxygen therapy uses a combination of increasing the atmospheric pressure and 100 percent oxygen to dissolve oxygen into the blood plasma and deliver it to body cells, tissues and fluids up to ten times the normal concentration. This reestablishes oxygen to body tissues that are compromised or have been receiving less than normal amounts of blood flow and promotes healing.

“We hope that hyperbaric oxygen therapy will stimulate the area around injured brain tissue to improve the patients’ cognitive functions,” said Dr. E. George Wolf, a staff physician in the SAMMC Hyperbaric Center. “We will also monitor symptoms of post traumatic stress disorder to see if there are any changes during the HBO study.”

Many patients are suffering from post traumatic stress disorder and traumatic brain injury and have symptoms of both, Dr. Wolf said.

The study will be conducted using 50 subjects who have been identified by their neurologists as having cognitive function problems and is scheduled to start in November. Potential subjects may be identified through neurologists at the Audie Murphy Veterans Hospital in San Antonio and SAMMC-North, or Brooke Army Medical Center at Fort Sam Houston, Texas.

According to a report prepared for Congress, more than 8,000 American military members currently suffer from some sort of brain injury as a result of the war on terrorism.

“It would be a great accomplishment if our study provides evidence that hyperbaric therapy can help these warfighters so they can be offered another opportunity to recover from their injuries,” Dr. Wolf said.

Until the 1990s few children were labelled as ‘autistic’ and it is clear that the recent epidemic is not because of better diagnosis, it is because some aspect of the medical management of children at birth or in early childhood has changed.

The blood vessels of the brain are different to those in the rest of the body; they form a barrier because many substances present in blood are toxic. The barrier, known as the blood-brain barrier or ‘BBB’ needs energy to work properly and so is affected by lack of oxygen. Brain damage due to failure of the BBB can occur at any time during life but especially, as MRI has shown, at birth. After an initial insult the barrier may remain damaged as it is, for example, in multiple sclerosis patients.

Medical practice lags well behind the latest research findings about oxygen from biological scientists and oxygen is simply regarded as necessary for the production of energy. However, it has been shown that oxygen levels control genes including those responsible for new blood vessel formation, the control of inflammation and the release of stem cells into the circulation. These processes apply to the repair of all tissues but especially the brain.

Breathing is necessary to gain oxygen for normal function but also for recovery from injury or illness. Unfortunately, injury and disease involve blood vessels and this often restricts the delivery of oxygen just when it is most needed for repair Misunderstandings about the toxicity of oxygen have generally clouded judgement about the use of high levels in treatment. Enormous experience exists in aviation, space and underwater medicine about the safety of using oxygen in treatment.

To significantly increase the level of oxygen carried in the blood needs a pressure chamber and it is well-established, if not well-known, that giving a high level of oxygen for just one hour a day may promote the repair of tissues when all other medical interventions have failed. In other words giving more oxygen extends the envelope of natural recovery.

There is no substitute for oxygen and if giving more does not help a patient it is not because oxygen does not ‘work’ it is because the damage has gone too far to allow normal recovery. However, the key question is how many treatment sessions are needed and this is a difficult problem because the treatment is time consuming.

The original studies of oxygen treatment for multiple sclerosis sufferers used a course of 20 sessions and this number was also used for the controlled studies. Over the last 27 years the UK charity centres have also used an initial course of 20 sessions followed by weekly maintenance sessions because MS is a progressive illness. A course of 20 sessions on a daily basis for five days a week means that a course can be completed in a month. If assessment shows continued improvement the course can be repeated after a suitable interval and most centres report using a gap of four weeks. The evidence supports using oxygen by hood at 1.5 ata or 1.75 ata using a mask. However benefit has been reported at pressures as low as 1.3 ata with 24% oxygen. March 2009″

Hyperbaric Oxygen Therapy (HPT) is a treatment that helps to increase the oxygen levels in organs and tissues. HPT is administered in a specialized pressure chamber. When receiving hyperbaric oxygen, you lie down in an enclosed capsule and the air pressure inside increases beyond that found in the atmosphere. This pressurizing effect allows the body to “absorb” more oxygen. Traditionally, HPT has been used for the treatment of conditions such as burns and wounds, decompression sickness and carbon monoxide poisoning.
Now, a new study published in the online journal BMC Pediatrics provides strong evidence for a new application to this veteran technology – the treatment of autism. Prior research hinted at the possible benefits of hyperbaric treatment in other neurological conditions such as brain injuries, cerebral palsy and fetal alcohol syndrome. There were also several “uncontrolled studies” and many anecdotal reports of success with HPT in autism. All these clues allowed for this larger, better controlled (more scientifically valid) study to take place.

This groundbreaking trial enrolled 62 autistic children with ages ranging from 2 to 7 years. The children were split up into two groups. Both groups received 40 one-hour treatment sessions in a hyperbaric oxygen chamber. One group received a higher level of oxygen (24%). This was considered the “treatment group”. The other kids received a lower level (21%). This second group was used as a control/placebo group. The outcome of the trial was measured using standardized autism symptom tests.

* The group that received the 24% oxygen showed improvements in several key symptoms, such as: maintaining eye contact, overall function, receptive language and social interaction.
* 30% of the treatment group was rated as “very much improved” or “much improved”. Only 8% of the control/placebo group was classified in the same manner.
* A total of 80% of the treatment group was considered “improved”. Only 38% of the control group met that same classification.

In addition, other benefits were noted when more specific testing was administered. Results from the Aberrant Behavior Checklist (ABC) and the Autism Treatment Evaluation Checklist (ATEC) revealed post-treatment improvements in the following areas:
* cognitive/sensory awareness
* hyperactivity
* irritability
* speech

You might wonder why the control/placebo group derived any benefit at all. There are two possible reasons. Firstly, the placebo effect is a real phenomenon. Secondly, even the control group received oxygen therapy. They just received less of it (21% vs. 24%). Therefore, it’s possible that even the lower dosage of oxygen was enough to provoke some benefit.

The authors of the study offered these concluding remarks, “Given the positive findings of this study, and the shortage of proven treatments for individuals with autism, parents who pursue hyperbaric treatment for their child with autism can be assured that it is a safe treatment modality at the pressure used in this study (1.3 atm), and that it may improve certain autistic behaviors.”

These new findings may very well herald a new day in the treatment of autism. Future research will help define the potential role of HPT in the treatment of autism and related disorders.
