The prostate is a small organ about the size of a walnut. It is found below the bladder (where urine is stored) and surrounds the tube that carries urine away from the bladder (urethra). The prostate makes a fluid that becomes part of semen. Semen is the white fluid that contains sperm.

Prostate problems are common in men age 50 and older. Sometimes men feel symptoms themselves, or sometimes their doctors find prostate problems during routine exams. Doctors who are experts in diseases of the urinary tract (urologists) diagnose and treat prostate problems.
There are many different kinds of prostate problems. Many don’t involvecancer, but some do. Treatments vary but prostate problems can often be treated without affecting sexual function.
Signs of Prostate Problems
• Frequent urge to urinate
• Blood in urine or semen
• Painful or burning urination
• Difficulty in urinating
• Difficulty in having an erection
• Painful ejaculation
• Frequent pain or stiffness in lower back, hips, or upper thighs
• Inability to urinate, or
• Dribbling of urine

If you have any of these symptoms, see your doctor right away to find out if you need treatment.
There are several prostate problems including:
• Acute prostatitis is an infection of the prostate caused by bacteria. It usually starts fast and can cause fever, chills, or pain in the lower back and between the legs. It also can cause pain when you urinate. If you have these symptoms, see your doctor right away. Antibiotic drugs usually help heal the infection and relieve the symptoms. Your doctor also may suggest that you drink more liquids.
• Chronic prostatitis is a prostate infection that keeps coming back time after time. Symptoms may be milder than in acute prostatitis, but they can last longer. Chronic prostatitis can be hard to treat. Antibiotics may work if bacteria are causing the infection. But if bacteria are not the cause, antibiotics won’t work. Massaging the prostate sometimes helps to release fluids. Warm baths also may bring relief. Often chronic prostatitis clears up by itself.
• Benign prostatic hyperplasia (BPH) is the term used to describe an enlarged prostate. BPH is common in older men. Over time, an enlarged prostate may block the urethra, making it hard to urinate. It may cause dribbling after you urinate or a frequent urge to urinate, especially at night. Your doctor will conduct a rectal exam to diagnose BPH. The doctor also may look at your urethra, prostate, and bladder.

• Prostate Cancer: Prostate cancer is one of the most common types of cancer among American men. It is more common among African American men than white men. Treatment for prostate cancer works best when the disease is found early.

These medications must be taken indefinitely or the prostate will start to grow again. Complications are minimal with the rare reported case of decrease in desire or loss of sexual ability.

An interesting side effect of the 5-alpha reductase inhibitors is reversal of baldness, a nice benefit for bald guys. On the other hand, few of the men I treat mention regrowth of hair.

Both Proscar and Avodart reduce PSA (prostate-specific antigen, measured for the blood test to detect early prostate cancer) by about half. After a period of a year, one can continue to monitor and screen for prostate cancer with this test, but it must be remembered that the values will be reduced. At the same time, Proscar has been shown to reduce the risk of prostate cancer by about 25%. Studies for similar effects by Avodart are ongoing, but I think it will also reduce risk.

Alternatively, alpha blockers, a class of medications sometimes used to treat high blood pressure, have been found to relax the muscle fibers within the prostate and outflow tube. This relaxation enlarges the channel and decreases the blockage. The medications do not shrink the prostate, and the prostate will continue to enlarge in size. They are, however, quite effective at reducing symptoms.

Flowmax is the most popular of the alpha blockers, followed by Uroxatrol. They have less tendency to reduce blood pressure than the old-line (and now generic) medications Hytrin and Cardura, but none of these medications is likely to kill two birds with one stone.
The symptoms will return immediately if the medication is stopped. Studies have shown a success rate of 40-50% in terms of a decrease in symptoms and improving urinary flow. Side effects include dizziness, fatigue, light headedness and fainting. For the first few days, the medication should be taken with caution at night, since when getting up to urinate there is the potential to pass out.

Nothing says that these two classes of medications can’t be used together, and that is, in fact, a common approach. A typical regimen after evaluation might be to start Flowmax and return in two weeks for further evaluation. If prostate cancer has been ruled out, PSA is over 2, and a digital rectal exam indicates enlargement, then I add Avodart. About a third of men can stop the Flowmax after three months, 60% can drop it at six months, and 90% are off within nine months. As noted previously, Avodart can also be reduced over time. especially considering the fact that most products work so poorly and help so little.

The first cause is hormonal. After about age 40 more of the 5-alpha-reductase enzyme is produced by men. This enzyme converts testosterone, which we all want to have more of as we age, to dihydrotesterone or DHT. As men age, DHT levels tend to raise because of this increased enzyme production. DHT causes the prostate to increase in size. Higher DHT levels are one of the major causes of enlarged prostates. However they are not the only cause. If they were, herbal formulas containing saw palmetto would work better than they do.

It seems that the majority of men suffering from enlarged prostates are not helped significantly by herbal formulas containing saw palmetto, pumpkin seeds and other herbs which block the formation of DHT and may inhibit its binding to cell receptors in the prostate. Most likely this is for two reasons. First is that these herbs don’t work as powerfully as their promoters would like us to believe. Secondly, they don’t deal with what may be an even more major cause of an enlarged prostate. Infections.

Our guess is that approximately 2/3 of all men suffering from an enlarged prostate have an infection in the prostate. This infection could be viral, bacterial, fungal or even mycoplasm. Infections tend to cause inflammation. An inflammation in the prostate enlarges it. And of course you may suffer from both an infection and increased DHT levels. Your doctor may be able to give you an antibiotic to help you with a bacterial prostate infection, but he has little to help with viral and mycoplasm infections.

For years this report has recommended unique products for improving the symptoms of an enlarged prostate. These products worked better than the usual saw palmetto and sterolin based herbal formulas.

Because they helped many people that were not helped by these usual types of herbal formulas. The first product was U-Fn, a u-fucoidan extract from brown algae that was expensive but better than the herbs. Unfortunately, many people still were not helped. So a few years later we heard about an oxygenating, detoxifying, pH balancing liquid supplement that fights infections. Zeo 3. For most patients suffering from an enlarged prostate who do not have kidney damage or serious infection, the ultimate decision must be left to the patient. While there are absolute indications of intervention, most situations depend on how bothersome the symptoms are to the individual. Sometimes men may take antibiotics for a long period of time without any true benefit. What is advanced prostate cancer?

Can advanced prostate cancer be cured?
Currently, no cure exists for advanced prostate cancer. Men can benefit, however, in several ways. Some treatments slow the growth of the cancer. Others reduce symptoms caused by the disease. Until recently, no therapy was offered to prolong life. However, the recently approved chemotherapy drug Taxotere, when taken with the steroid prednisone, has been proven to extend survival by several months. Taxotere is approved only for men with cancer that no longer responds to hormone therapy (called hormone refractory prostate cancer).

In addition, Provenge, a “”vaccine”" for the treatment of advanced prostate cancer, does appear to extend survival — by months for most patients, by years for some. Provenge isn’t your everyday vaccine. It’s an immune therapy created by harvesting immune cells from a patient, genetically engineering them to fight prostate cancer, and then infusing them back into the patient. It’s approved only for treatment of patients with few or no symptoms who have prostate cancer that has spread outside the prostate and no longer responds to hormone therapy.

Be wary of spicy foods. Spicy and acidic foods bother some men with enlarged prostates, says Dr. Neal. “”If you notice more problems after eating salsa, chili or other spicy or acidic foods, then you’re among those men–and you should avoid that cuisine.

Manage your stress. Saw palmetto is the leading herbal treatment for BPH and among the best selling of all herbal products, with annual U.S. sales of more than $25 million, according to the American Botanical Council.

“Saw palmetto is usually the first line of therapy against BPH — at least by patients or their spouses,” quips Franklin Lowe, MD, MPH, associate director of the Department of Urology at St. Luke’s/Roosevelt Hospital and chairman of the American Urological Association’s Complementary & Alternative Medicine Committee. “Most patients are taking it before they come to their urologist’s office to find out if they should be taking it.” Still, many urologists endorse it use for patients with early, troubling symptoms.

Pros: This herb brings relief in about one in three patients, with a suspected effect similar to Proscar, by blocking the production of a hormone that causes the prostate to grow. “Nobody really knows exactly why it works, because studies are conflicted on the suspected mechanism,” says urologist Eric K. Seaman, MD, FACS. “What is clear is that it helps a lot of people, and there is no documented evidence of any side effects. I know a urologist who was waking up at least three times a night because of BPH, and since taking saw palmetto, he’s up only once a night. So even if the majority of patients don’t get a benefit, they don’t lose anything either other than a few dollars.”

Cons: One possible explanation for why the majority of patients don’t get relief? The active ingredient of saw palmetto — like its exact mechanism — hasn’t been identified, so different formulations may produce different results, says Katz.
Red clover has long been used as an alternative treatment for a variety of conditions — from menopausal hot flashes to psoriasis — because it is rich in isoflavones, a protein found in soybeans. Not only is this believed to help inhibit prostate and other cancer tumors, it could help treat BPH. A product commercially sold as Trinovin (and whose manufacturer paid for Katz’s study) has been found in several trials to improve urine flow and decrease symptoms by as much as 23%.
Pros: Isoflavones are suspected of helping more than BPH, but Trinovin contains four types believed to be powerful for overall health and in treating BPH and reducing prostate cancer risk.

Cons: Some studies suggest that improvements are most noticeable after one month and then continue at a much slower rate. Little or no improvement is noticed at varying doses.
Soy contains high concentrations of the same isoflavones found in red clover and is often cited as a reason why Japanese men rarely get prostate cancer.

Pros: Also believed to offers many health benefits and strong evidence of reducing prostate cancer.
Cons: For maximum benefit, it’s recommend that men consume eight ounces of soy foods daily, which is impractical and would create severe gas problems, says Katz.

African tree bark (pygeum africanum) is so popular in Europe and Africa that the tree from which this treatment is harvested is now on the endangered list.

Pros: Studies show it brings relief, but how? Some show it relieves symptoms by reducing prostate swelling, while others indicate it may reduce inflammation or inhibit growth factors associated with prostate enlargement and tumor development.
Cons: Typically not a standalone treatment, and usually used in conjunction with saw palmetto.

Of course, because they are not regulated by the FDA, some experts say that many herbal supplements vary widely in the amount of active herbal extract claimed on the label. A study presented to the American Urological Association last year found that some brands tested contained less than 15% of the active ingredient listed on their label.

THEORIES FOR BPH

BPH can be called an aspect of male menopause because an increased ratio of estrogen to testosterone is active in BPH just as, conversely, in women passing through menopause the ratio of testosterone to estrogen increases. It is generally accepted that hormone ratios and hormone clearance are involved in BPH, but the exact ways in which these lead to the enlargement has yet to be definitively explained. Testosterone, the “male” hormone, is at its peak during adolescence. It decreases thereafter, and the rate of decrease sharpens by about age 50. The decline in testosterone production typically calls into play the compensatory release of other hormones which are stimulants to testosterone production.

Forget about those yearnings for red convertibles and shapely young blondes. The real midlife crisis occurs in a man’s prostate, the gland that adds fluid to semen so that he can ejaculate. Four of every five men over age 50 develop an enlarged prostate–or, more specifically, a condition called benign prostatic hyperplasia (BPH). One-fourth to one-third of them will experience BPH’s uncomfortable and potentially dangerous symptoms.

“BPH causes no pain, but it does make urination more difficult,” says Stephen Rous, M.D., professor of surgery at Dartmouth Medical School and a urologist at Dartmouth-Hitchcock Medical Center in Lebanon, New Hampshire. Because the prostate surrounds the urethra, the tube that carries urine from the bladder, when it enlarges it restricts urine flow. This results in a need to urinate more frequently, often with increased difficulty getting started.

With prostate problems, you may also experience dribbling, because the prostate isn’t as strong as it used to be and you can’t urinate with the same force. Some men with this problem are unable to sleep through the night without waking to urinate, while others are completely unable to urinate–an emergency condition.

Surgery to remove the prostate is one alternative, and there are several medications–some of which take months to work–that can reduce an enlarged prostate and improve urination. But for tried-and-true home treatments, here’s what the experts recommend.

Cut the caffeine. “Caffeine in any form–coffee, tea, chocolate or soft drinks–tends to tighten the bladder neck and make it more difficult to pass urine,” says urologist Durwood Neal, Jr., M.D., associate professor of surgery, urology, microbiology and internal medicine at the University of Texas Medical Branch at Galveston. “Some of the prostate is made up of smooth muscle, and anything that causes that muscle to constrict will make urination more difficult. Caffeine does this quite a bit.
