Susan Rodriguez, a certified hyperbaric specialist from San Bernardino, California, believes that to understand how HBOT works for CRPS, you need to understand the disease itself. “CRPS is neurological in nature and yet it manifests itself in physical symptoms,” she says. “What that means is that the disease needs to be treated with two approaches. One goal is to restore circulation, reduce inflammation or edema, and remove the swelling in affected limbs so the limbs can live. The other goal—if you want to eliminate the disease—is to work on the brain.”

If you were to stub your toe, for example, the central nervous system would tell your brain that your toe hurts. This pain is rooted in a physical injury. But sometimes trauma and inflammation mysteriously trigger a reaction from the sympathetic nervous system, which is a different part of the brain. With CRPS, pain is read through sensors in the sympathetic nervous system. (The sympathetic nervous system is what is activated in phantom leg pain, for example.) “Hyperbaric Oxygenation Therapy, however, can make the switch in the brain back to the central nervous system,” Rodriguez says. Under a doctor’s direction, she treats both the affected limb and the brain by different atmospheric pressures when the patient is inside the chamber.

“Different undersea depths work on different parts of the body,” she explains. “Deeper depths (up to 33 to 45 feet undersea) work more on tissue and bone, while milder hyperbarics (such as 18 to 24 feet undersea) work on the brain. Since we are working on both things, I take patients to all those levels. Almost always, the first symptom to come is the last to go. And then the symptoms are gone!”

Rodriguez learned about the effectiveness of HBOT therapy in treating CRPS first hand, when her husband Patrick was diagnosed in the mid-1990s following surgery. Because she had already been working in this field, they decided to try Hyperbaric Oxygenation Therapy to treat his CRPS. Not only is Patrick working today, but he has become a certified hyperbaric technician. The couple opened Rapid Recover Hyperbarics in 1998, with Donald Underwood, DO, MD, JD, serving as medical director.

A physician should review all aspects of a patient’s medical history before starting therapy. There are a few medical conditions that may prevent an individual from receiving HBOT — either permanently (certain lung and heart disorders) or temporarily (sinus infections, fevers).

One day I was talking to a mother whose daughter had CP and she told me about a study being conducted. I called the physicians doing the study and enrolled my daughter.

Elisa received her first 40 treatments when she was 3 ½ years old. At that time she was able to mumble about 35 words which were understood mostly by myself only. She could say two words together like bye-bye Mommy! She was able to say “meek” for “I’m thirsty” and would be hysterical when she was hungry. She never watched a program like Barney or Sesame Street- she could never focus on them. After 115 dives Elisa is now completely toilet trained. She can say over 1,000 words. She not only is able to watch a tape but asks for specific Barney tapes that she likes. She can count to six, knows her colors and can make herself understood. We do have a lot to accomplish still, but she is nothing short of a miracle.

My first daughter is right. If Elisa did not have a handicap I would have never taken a second mortgage on my house and bought a chamber. I would never be leaving to go to California! But look what we got! We have a child who is witty and happy and can brighten any room she enters! Last week she said “I ove you!” She adores her 2 sisters and 2 brothers and tries to imitate them! WE also have four other children who are sensitive and caring. They treat their sister with respect and love. Even the triplets who are three understand that when Elisa calls we have to respond to her and help her when she asks. They fight like all siblings do but they all stand united when threatened by even their parents. They tell on each other but are always willing to keep the child on “time out” company.

“We thanked Dr. Hernandez for saving Kevin’s life. He responded that it was God that had saved Kevin and that he was simply one of God’s instruments. Kevin stayed in the PICU at NSRMC for 32 days. He was taken off of the three antibiotics after 14 days. During his stay in the PICU he developed several complications including acute hydrocephalus so a shunt had to be put into place. Four attempts at extubating Kevin failed and he had to have a tracheotomy and he was dependent on the ventilator to breathe throughout his stay. A feeding tube was put in place so Kevin could get his nourishment. His gall bladder became inflamed and he was put on medication.

He had a few seizure episodes in the beginning but they resolved. The last brain scan showed severe brain atrophy. He suffered partial hearing loss in his left ear but his right ear was spared. His vision was a question. An ophthalmologist determined that Kevin was possibly blind.

Kevin’s progress and recovery is a story all unto itself. Dr. Hernandez is a young doctor that aggressively treats his patients. On the 8th day after Kevin entered the hospital, he approached us about wanting to put Kevin into a Hyperbaric Oxygen Chamber. Hyperbaric Oxygen Therapy (HBOT) is most often associated with scuba divers experiencing decompression sickness.

The patient is put into a chamber with 100% compressed oxygen. HBOT is old technology but being put to new uses. Sadly, the FDA has only approved its use for only a small number of applications including non- healing wounds. There are not enough studies for brain injuries at this time for FDA approval. “”Fortunately”" for Kevin, he had developed a decubitus on the back of his head. This was Kevin’s ticket to the hyperbaric chamber. Dr. Hernandez assured us that Kevin would be in no danger at any time. While the HBOT is healing the decubitus, it could potentially help to heal his brain. There were no guarantees because not all patients respond to Hyperbaric therapy.

Dr. Hernandez explained the theory of HBOT and how it could help to heal Kevin’s brain. At the center of the infarct, all the cells are dead and nothing can be done to change that. However, there is area between the center of the infarct and the healthy cells called the Penumbra. The cells in the penumbra are not dead but dormant. They cannot receive good blood flow to make them function. Hyperbaric in theory forces the compressed oxygen to those dormant cells, awakens them, and through some unexplained mechanism, proper blood flow is re-established to that area. The cells again become functional and it is hoped that they create a path around the damaged areas of the brain. HBOT also helps promote plasticity.

Dr. Hernandez further explained that he is successfully treating children with Cerebral Palsy and other patients that had sustained brain injuries years earlier. He was optimistic that Kevin would respond because Kevin was still in the acute phase of the illness. As parents, we had nothing to lose. HBOT would not hurt Kevin and it had the potential to help him. We agreed to try Hyperbaric therapy. On the 9th day Dr. Hernandez ordered tubes put in Kevin’s ears so the pressure in the chamber would not damage them. Kevin took his first “”dive”" in the chamber on day 10. He was on the ventilator and to our knowledge, he is perhaps the youngest patient to go into a hyperbaric chamber.

Dr. Hernandez initially ordered 70 dives and at the end of those 70, he would evaluate the progress and order more dives as necessary. The dives last one hour each day. During the third dive Kevin opened his eyes for the first time. Kevin continued to get stronger and soon Colleen or I had to go into the chamber with him to restrain and comfort him. Kevin began to amaze his doctors and therapists. The improvements and change showed with each passing day. After only a few dives, three of the five infarcts were no longer visible on the CT scans.

One of Kevin’s infarcts was located at the occipital lobe. This area of the brain controls vision. By the time Kevin was discharged, his vision had returned and he was no longer blind. Regarding his left side weakness, Kevin slowly began using his left hand. He now has full use of his left hand and can wiggle his toes on his left foot. Kevin shows no apparent weaknesses and has virtually reached his baseline. He feeds himself, crawls, plays with his toys, and should be walking very soon. He is doing age appropriate developmental tasks.

Dr. Hernandez and others familiar with his case are predicting that Kevin should make a full recovery. Dr. Hernandez and Dr. Pena-Miches never dreamed that Kevin would come this far and certainly not this fast. Kevin has reached 81 dives to date and has orders to 120 dives. He will be evaluated again at that time. Our family was blessed with many gifts throughout this terrible experience. We were given back the gift of our son and our family was restored. It appears that Kevin will make a full recovery in spite what shows on his brain scans.

We are told he should grow up and live a normal life and this whole episode could be nothing more than a painful memory for us. At six years old he is a normal boy with only a slight speech delay. Kevin’s doctors often refer to Kevin as a miracle. Kevin is our miracle and he certainly is what his sisters call him, “”Kevin from Heaven”".

Colleen and I along with Kevin’s nurse, Karen Boyd, are on a crusade for HBOT. We want to generate as much awareness as we can about HBOT. Kevin was fortunate to have had a doctor familiar with the potential benefits of HBOT and was determined to get Kevin in the chamber quickly.

Kevin received HBOT while still in the acute phase. Perhaps that explains his rapid and successful recovery. It is hoped that our experience will help other parents and provide a treatment option they might not have considered.

Dr Murray also had admitting privileges at brooks. He made a phone call to brooks and they sent someone to st. Vincent’s again to evaluate sherry. They told us again she wasn’t a candidate but realized we weren’t taking no for an answer. They agreed to a ten-day trial and told us she would never tolerate the program. That 10 days was later changed to 3 months when she began making progress. Sherry was so close to walking on her own and trying so hard. We continued our trips to hyperbaric treatments two days a week. These treatments cost 200.00 each. When our family had depleted savings and retirement accounts we didn’t know how we would continue her treatments.

Our parents were considering taking a loan out on their home. I knew they couldn’t afford to do that so again I was on my knees asking god to lead the way. Once again he came through for sherry. We began having fundraisers. we have had 3 gospel sings, sold shrimp, taken photographs for donations, held cake auctions, raffles,sent her story to numerous newspapers, told her story on the gospel radio station, and had numerous yard sales. Sherry has had 172 treatments to date. We have fought with her insurance company along the way. sherry was walking with a walker when her insurance company refused to pay for anymore physical therapy.

I have written numerous letters to the Governor of Florida to no avail trying to get her some help. we took sherry to a handicapp accessible gym for awhile and now she goes to a local college for physical therapy. the devil has tried so hard to destroy our faith, but I know no matter how hard he tries God will make a way. sherry has overcome to many obstacles. Sherry is already a miracle beyond belief. Just to know that she can understand us, respond to us, hug and kiss her family and on occasions we get to hear her actually voice I love you.

We began searching for a miracle and discovered hybaricic treatments. these treatments were still experimental but had amazing results in persons with brain injury. the doctors told us we would only waste our money and time. I began researching these treatments and was able to find one doctor that specialized in these treatments for brain injury. we contacted him on the phone and explained our situation. The doctors at st. Vincent’s were pushing us to put her in a nursing home. He told us to bring sherry to his office for a consult. He didn’t know if we would be able to convince the doctors to allow us to transport her. I told him I would contact him in one week to set up a consult visit. With further research we realized these treatments were not FDA approved for brain injury.

This meant sherry’s insurance wouldn’t cover the cost of the treatments or for her transportation. the treatments cost 200.00 each and we would need a handicapped van and stretcher to transport her. That didn’t stop us we began searching for a handicap van. We were able to find one in great condition for 2500.00. She was unable to sit upright. I borrowed a stretcher from Irwin co. Ems. We took the stretcher to make sure we had enough clearance. It worked perfect but we discovered another problem what was going to keep it from turning over.My husband said” follow me to home depot”. He came out with some ratchet straps. He used the straps and tied the stretcher down to the rods that normally holds the back seats. He said get up there and buckle up were going for a test drive. It worked like a charm. Then came our next hurdle, we had to convince the doctors to allow us to transport her to Dr. Murray’s office.

They told us we had to sign a release so they wouldn’t be held responsible if something happened. Lord I prayed we were doing the right thing. When we got to Dr. Murray’s office we realized the rehabilitation center we wanted sherry to go to was only a block away. I called my mom and told her how close brooks rehab was to Dr. Murray’s office. When we met Dr. Murray he told us he had a son that suffered from a brain injury and has dedicated his life and practice to brain injuries. Brain injury was his expertise. I knew once again god was answering our prayers and god was in control of the situation.

July 8,2003 Sherry was admitted to St. Vincent’s Hospital with suspected meningitis. The Doctors didn’t give us much hope. She was in a vegetative coma by the second night. We were told the intracranial pressure caused severe brain damage. We were told both basal ganglia were gone and that was the part of the brain responsible for transmitting signals from the spinal cord to the brain for all motor functions. We were in shock , this couldn’t be sherry was only 33 and had two young boys to raise. We asked for a second opinion. We consulted the best neurology group in jax. Florida from shands. They told us the same thing.

The brain was severely damaged both basal ganglia were gone and part of the brain stem was damaged. He did tell us that there was a lot that was not yet understood about the brain and only the good man above knew what the outcome would be. Medically speaking though he needed to tell us that she would never regain the ability for any motor functions without a miracle. This meant she would never talk, walk, sing, dance, and worst of all never hug her two children again. She remained comatose for over 3 months. she was on the ventilator, had a tracheostomy tube, feeding tube and unresponsive.

Hyperbaric treatments makes perfect sense. Increased pressure in the chamber causes blood vessels in the brain to constrict, which eases the swelling and allows more oxygen into the brain itself. Supposedly the combination of higher pressure and more oxygen can activate previously dormant areas of the brain and even make the brain regenerate tissue more quickly.

The only study I’ve found online does make it sound like a good thing to explore at the very least, assuming you have the resources to do it. The study took sixty two kids with autism (ages 2-7) and put them into the hyperbaric chamber about an hour and a half per day, equaling out to a total of forty hours spread out over one month. Half of the kids got a 24% oxygen at an atmosphere of 1.3 atm (whatever that means) and the other half got plain old air in a barely pressurized room.

80% of the treatment group (the ones who got the 24% oxygen) showed significant improvement in eye contact, behavior, cognitive and sensory awareness, and receptive language. 38% of the control group showed improvement.

30% of the treatment group were pronounced “much improved” by their doctors, vs only 8% in the control group.

Hyperbaric oxygen (HBO) therapy is a medical treatment that uses the administration of 100 percent oxygen at a controlled pressure (greater than sea level) for a prescribed amount of time—usually 60 to 90 minutes.

The air we normally breathe contains 21 percent oxygen, 78 percent nitrogen, and 1 percent carbon dioxide and all other gases. Increasing the pressure of the air we breathe through the administration of HBO therapy has the effect of dramatically increasing the number of oxygen molecules entering the bloodstream, up to 2000 percent. This increase in oxygen under controlled pressure means that oxygen can reach bone and tissue that is normally not accessible to red blood cells. Red blood cells carry oxygen throughout the body.

HBO therapy affects the body in other ways. For example, it enhances the function of white blood cells, which fight infection. This is important because increasing the efficiency of white blood cells through the use of HBO therapy has a positive effect on the immune system. Also, HBO therapy has a positive effect on peripheral blood vessels and causes the formation of new capillaries, which are tiny, thin-walled blood vessels.
