Hyperbaric Oxygen Therapy (HBOT) is a specialized medical intervention in which the atmospheric pressure is increased above 1 ATA (above sea level-14.7 psig), usually by the filling of a single person chamber, multi-place chamber or a hyperbaric room with a dose specific amount of oxygen (atmospheric oxygen or enriched oxygen). At sea level, our lungs absorb a certain amount of oxygen molecules from the air. When descending to lower altitudes (below sea level), the pressure is greater (above 1ATA) and now the lungs more easily absorb the compressed oxygen molecules in the air.

In contrast, at higher altitudes, when the pressure drops, the lungs work harder to absorb the dispersed oxygen molecules from the air. This is why oxygen masks deploy in an airplane during sudden high altitudes changes, helping to increase the O2 content due to a lack of pressure.

During a hyperbaric “dive” the fluids and tissues of the body receive an infusion of readily available oxygen. In fact, even cells and areas of the body with limited circulation become saturated in oxygen. The effect is an uptake of oxygen in the blood, plasma cerebral-spinal fluids, and tissues. In addition, the vaso-constrictive nature of hyperbaric therapy has an added effect of reducing inflammation and edema.
Therapy Options

There are many different manufacturers and designs of HBO chambers–monoplace, multiplace, and portable. The treating physician’s protocol will dictate which type of chamber should be used.
Uses of HBOT

Currently, there is an increase in the amount of research being conducted on hyperbaric medicine in modern practice. Published studies in leading journals, such as The New England Journal of Medicine document the use of HBOT for the treatment of such indications as multiple sclerosis, acute carbon monoxide poisoning, gas gangrene, air embolism, diabetic foot wounds, infections, burns, ulcers and edema. But these extablished indications only begin to open the current chapter in the use of modern hyperbaric technology.

Hyperbaric Oxygen Therapy

Hyperbaric Oxygen Therapy (HPT) is a treatment that helps to increase the oxygen levels in organs and tissues. HPT is administered in a specialized pressure chamber. When receiving hyperbaric oxygen, you lie down in an enclosed capsule and the air pressure inside increases beyond that found in the atmosphere. This pressurizing effect allows the body to “absorb” more oxygen. Traditionally, HPT has been used for the treatment of conditions such as burns and wounds, decompression sickness and carbon monoxide poisoning.
Now, a new study published in the online journal BMC Pediatrics provides strong evidence for a new application to this veteran technology – the treatment of autism. Prior research hinted at the possible benefits of hyperbaric treatment in other neurological conditions such as brain injuries, cerebral palsy and fetal alcohol syndrome. There were also several “uncontrolled studies” and many anecdotal reports of success with HPT in autism. All these clues allowed for this larger, better controlled (more scientifically valid) study to take place.

This groundbreaking trial enrolled 62 autistic children with ages ranging from 2 to 7 years. The children were split up into two groups. Both groups received 40 one-hour treatment sessions in a hyperbaric oxygen chamber. One group received a higher level of oxygen (24%). This was considered the “treatment group”. The other kids received a lower level (21%). This second group was used as a control/placebo group. The outcome of the trial was measured using standardized autism symptom tests.

* The group that received the 24% oxygen showed improvements in several key symptoms, such as: maintaining eye contact, overall function, receptive language and social interaction.
* 30% of the treatment group was rated as “very much improved” or “much improved”. Only 8% of the control/placebo group was classified in the same manner.
* A total of 80% of the treatment group was considered “improved”. Only 38% of the control group met that same classification.

In addition, other benefits were noted when more specific testing was administered. Results from the Aberrant Behavior Checklist (ABC) and the Autism Treatment Evaluation Checklist (ATEC) revealed post-treatment improvements in the following areas:
* cognitive/sensory awareness
* hyperactivity
* irritability
* speech

You might wonder why the control/placebo group derived any benefit at all. There are two possible reasons. Firstly, the placebo effect is a real phenomenon. Secondly, even the control group received oxygen therapy. They just received less of it (21% vs. 24%). Therefore, it’s possible that even the lower dosage of oxygen was enough to provoke some benefit.

The authors of the study offered these concluding remarks, “Given the positive findings of this study, and the shortage of proven treatments for individuals with autism, parents who pursue hyperbaric treatment for their child with autism can be assured that it is a safe treatment modality at the pressure used in this study (1.3 atm), and that it may improve certain autistic behaviors.”

These new findings may very well herald a new day in the treatment of autism. Future research will help define the potential role of HPT in the treatment of autism and related disorders. Hyperbaric Oxygen Therapy (HBOT) has been used more and more frequently in the past decade to treat Lyme disease. A ground breaking study published by Dr. William Fife of Texas A&M University, demonstrated dramatic improvements in overall condition of Lyme patients treated with hyperbaric. Hyperbaric has been shown to reduce pain significantly, modulate the immune system, increase energy, alleviate sleep dysfunction, and reduce cognitive impairment. In most cases, patients are also able to discontinue use of antibiotics or other pharmaceuticals.

Hyperbaric oxygen treatments infuse the body with oxygen, increasing O2 levels by up to 1000% in body tissues through the increased pressure. Borrelia burgdorferi, the bacterium that causes Lyme are considered microaerophilic, needing small amounts of oxygen to survive, but dies in the presence of abundant oxygen. Hyperbaric also acts as an immune modulator and allows organ and gland functionality to normalize, reducing many debilitating Lyme symptoms. Because the chambers pressurize the atmosphere, hyperbaric acts as a detoxifier as well. By forcing oxygen into the tissues through this pressure, toxins, chemicals and other impurities are forced out.

As with antibiotic treatment, patients may experience a Jarisch-Herxheimer reaction, after initial hyperbaric oxygen treatment. HBO was used in 830 newborn infants: in 555 infants with asphyxia, in 165 infants with syndrome of respiratory disturbances (SRD) and 110 infants with the aspiratory syndrome (AS). During the treatment of asphyxia an early use of HBO, 1-5 min after artificial pulmonary ventilation (APV), as compared with a late use of HBO, 10-30 min after APV, leads to a decrease of cerebral circulatory disorders by 4 times, and or mortality rate by 8 times. During the treatment of SRD, the employment of HBO in the First 1-3 hrs of life led to recovery of 75% of infants. The delayed use of HBO, 12-48 hrs after birth is ineffective. HBO applied in the first hour of life during the treatment of AS prevented the development of aspiratory pneumonia in 92.7% of cases.

HBO should be used during the treatment of neonatal asphyxia in combination with APV, infusion “alkalizing” therapy, and during the treatment of SRD and AS it should be combined with cardial, anti-bacterial, infusion, “alkalizing” therapy. The duration of sessions should not be less than 1.5-2 hrs and not more than 3 hrs at 2-3 ATA for 10-15 min and at 1.4-1.5 ATA for 1.5-2.5 hrs. When these conditions are met, acid-base balance and blood gases normalize. If necessary, such sessions are repeated in 6-9 hrs; to delay them for 12-30 hrs is not advisable. Thus, effectiveness of HBO during the treatment of acute hypoxic states in new-born infants depends on its early, complex and repeated application under safe resuscitation and therapeutic regimes.

http://www.hbot.com In the past two years a disturbing trend has become a standard of care in the off-label use of HBOT for chronic pediatric brain injury, the use of 1.75 ATA oxygen. Unfortunately, this is merely the latest re-emergence of the age old illusion and nemesis of HBOT in chronic brain injury: if lower pressure is effective, surely higher pressure is more effective. This is the cruel self-defeating trap/paradox of HBOT in chronic brain injury, the misguided thinking that has plagued hyperbaric physicians since the 1960′s, and the primary reason for the lack of progress of HBOT in brain injury until the recent adoption of 1.5 ATA. It also explains why patients with chronic stroke, traumatic brain injury, and other neurological conditions who have been treated with HBOT at or above 2.0 ATA for wound care in the past 40 years don’t experience simultaneous regression or improvement of their neurological symptomatology.

The fact is, in the modern era of HBOT attempts to treat chronic brain injury with HBOT have repeatedly failed due to physicians’ inability to accept the fact that injured brain is exquisitely responsive to HBOT at pressures around 1.5 ATA. Whenever doctors have been able to resist the allure of higher pressure and the siren call of “MORE IS BETTER”, treatment has succeeded. Multiple sclerosis is the best example. History repeats itself. We are now making the same tragic mistake.

The key to the misperception of MORE IS BETTER is rooted in the failure to realize that HBOT is a drug. Like all drugs, oxygen at pressure has a therapeutic window; too low a dose is ineffective and too high a dose is both ineffective and toxic. In addition, too frequent, too long at depth, and too many treatments in a row are also toxic. For example, the well-known cardiac drug digoxin is ineffective at low dose, powerfully effective at the right dose, and lethal at just a smidgen higher dose. The anti-coagulant (blood thinner) Coumadin is the same, as are all inhalational anesthetics which can and will kill people at doses barely above the therapeutic dose. HBOT is no different.

The brain is similar to a large spear of broccoli. The broccoli fronds are like the outer part of the brain, the cortex or grey matter, where the brain cells exist and the spear or stalk is like the connecting tracts in the center of the brain. These tracts are also known as the white matter. Trauma causes an injury to the brain that is mechanistically identical to shaking the broccoli by its stalk. The shaking causes a mechanical disruption of the connecting tracts between brain cells. These tracts are similar to the cable that carries our cable television and broadband. Once disrupted cells can’t communicate between themselves and some of the cables die back to the cells which then die.

Each cable that is lost causes a loss of neurological function and thinking. In addition, each little site of disruption of cable or blood vessels is a wound in the brain that starts the inflammatory reaction. It is also associated with swelling, just like a swollen ankle or a bruise on the arm. As swelling occurs the swelling fluid compresses the small blood vessels in the area leading to low blood flow and low oxygen. Nearby cells then are further injured and live in a dormant state, or die.

This is known as the “vicious cycle” of traumatic brain injury. The net result is loss of neurological function and symptoms such as headache, confusion, problems with memory, attention, concentration, sleep disturbance, irritability, etc.

Why HBOT for brain injuries? Why NOT HBOT for brain injuries? HBOT has been used for nearly 50 years for new and old wounds in the body regardless of the location in the body. New wounds such as “flesh eating bacteria” infections or wounds whether they are on the torso, leg, groin, or neck, gas gangrene from war wounds in the same locations, crush injuries and blast injuries from explosions regardless of location, and burns to any part of the body are all treated with HBOT. Similarly, old wounds caused by infection to any of the bones in the body, diabetic foot wounds and other long-term ulcers of the feet, and old radiation wounds in the jaw, neck, chest, abdomen, or anywhere else in the body are responsive to HBOT.
