Lymphokinetic motion (flow of the lymph) due to:

1) Lymph flows down the pressure gradient.
2) Muscular and respiratory pumps push lymph forward due to function of the semilunar valves.

Other lymphoid tissue:

1. Lymph nodes: Lymph nodes are small encapsulated organs located along the pathway of lymphatic vessels. They vary from about 1 mm to 1 to 2 cm in diameter and are widely distributed throughout the body, with large concentrations occurring in the areas of convergence of lymph vessels. They serve as filters through which lymph percolates on its way to the blood. Antigen-activated lymphocytes differentiate and proliferate by cloning in the lymph nodes.

2. Diffuse Lymphatic Tissue and Lymphatic nodules: The alimentary canal, respiratory passages, and genitourinary tract are guarded by accumulations of lymphatic tissue that are not enclosed by a capsule (i.e. they are diffuse) and are found in connective tissue beneath the epithelial mucosa. These cells intercept foreign antigens and then travel to lymph nodes to undergo differentiation and proliferation.

Local concentrations of lymphocytes in these systems and other areas are called lymphatic nodules. In general these are single and random but are more concentrated in the GI tract in the ileum, appendix, cecum, and tonsils. These are collectively called the Gut Associated Lymphatic Tissue (GALT). MALT (Mucosa Associated Lymphatic Tissue) includes these plus the diffuse lymph tissue in the respiratory tract.

3. The thymus: The thymus is where immature lymphocytes differentiate into T-lymphocytes. The thymus is fully formed and functional at birth. Characteristic features of thymic structure persist until about puberty, when lymphocyte processing and proliferation are dramatically reduced and eventually eliminated and the thymic tissue is largely replaced by adipose tissue.

The lymphocytes released by the thymus are carried to lymph nodes, spleen, and other lymphatic tissue where they form colonies. These colonies form the basis of T-lymphocyte proliferation in the specific immune response. T-lymphocytes survive for long periods and recirculate through lymphatic tissues.

There are portable Hyperbaric chambers, which are used for home treatment. These are usually referred to as “”mild hyperbaric chambers””, which is a reference to the lower pressure of soft-sided chambers. Those commercially available in the USA go up to 4.1 PSI which is equivalent to a water depth of 11 ft. These chambers are operated with oxygen concentrators or with 100% oxygen as the breathing gas.

These portable hyperbaric chambers were originally developed and used for altitude sickness. The benefits of Hyperbaric Chambers to a number of other off label conditions such as autism, lyme disease, cancer, stroke, cerebral palsy, multiple sclerosis, cardiac conditions, brain injury, and other immunodeficiency disorders do have research and case studies to substantiated the effectiveness.

Recent studies have also shown that hyperbaric at a lower pressure for a period of 6 months or more is more effective for most conditions as the mild chambers have an immune modulating effect and hyperbaric at a higher pressure has a tendency to be immune suppressive over time.

“What very few patients seem to realize is that the immune system does not fight cancer so long as there is infection in the body. Isn’t this one of Nature’s ironies? People are more afraid of cancer than infection, but the body tries to eliminate infection before turning its resources towards cancer control.

The lymphatic system in many people is overwhelmed by infection, often stemming from dental problems, as well as toxic substances (mercury, aluminum, chemicals from antiperspirants, dry cleaning, and so forth.) Lending a hand to the lymphatic system will usually relieve some of the pressure on other parts of the body.

Functions of the lymphatic system:
1) to maintain the pressure and volume of the extracellular fluid by returning excess water and dissolved substances from the interstitial fluid to the circulation.
2) lymph nodes and other lymphoid tissues are the site of clonal production of immunocompetent lymphocytes and macrophages in the specific immune response.

Filtration forces water and dissolved substances from the capillaries into the interstitial fluid. Not all of this water is returned to the blood by osmosis, and excess fluid is picked up by lymph capillaries to become lymph. From lymph capillaries fluid flows into lymph veins (lymphatic vessels) which virtually parallel the circulatory veins and are structurally very similar to them, including the presence of semilunar valves.

The lymphatic veins flow into one of two lymph ducts. The right lymph duct drains the right arm, shoulder area, and the right side of the head and neck. The left lymph duct, or thoracic duct, drains everything else, including the legs, GI tract and other abdominal organs, thoracic organs, and the left side of the head and neck and left arm and shoulder.

These ducts then drain into the subclavian veins on each side where they join the internal jugular veins to form the brachiocephalic veins.

Lymph nodes lie along the lymph veins successively filtering lymph. Afferent lymph veins enter each node, efferent veins lead to the next node becoming afferent veins upon reaching it.

Hyperbaric oxygen is presently being used in an attempt to improve functional outcome following a multitude of brain injuries such as stroke, anoxic brain injury, traumatic brain injury, and others. Family members of brain-injured individuals correspond via the Internet into coalitions demanding that their loved ones receive hyperbaric oxygen therapy. There are anecdotal reports of benefit with hyperbaric oxygen following brain injury but there is little credible scientific evidence for efficacy of hyperbaric oxygen in brain injury.

Clearly, what is needed to answer if hyperbaric oxygen improves brain injury are results from carefully designed multi-center, prospective, randomized controlled clinical trials. However, the implementation of such a Phase III clinical trial is challenging without information from Phase II trials. We propose to conduct a feasibility trial that may potentially guide a future Phase III clinical trial..

Lymphatic System Cleansing

As all patients who have had lymph nodes removed know, circulation is generally worse after the loss of nodes: swelling, serious edema, and the need for medical procedures to remove fluids are all part of the aftermath of these procedures.

I am not in a position to evaluate the merits of the surgical measures. I merely sympathize with the suffering of the patients. They often have reddish swelling, intense itching, rashes, and loss of motion.
There are specialists in manual lymphatic drainage, a very delicate massage technique taught in Austria (called the Vodder method) who are trained to help reroute lymph flow so as to by-pass the normal channels, the ones that have been removed.

Though people who are specialists in this method are reluctant to state opinions, many have theorized that when malignancy is found in the lymph that the lymph nodes are doing exactly what they are supposed to be doing. In other words, it is not 100% clear to them that the cancer has metastasized, merely that it has accumulated in the lymph.

Given that we do not understand these fine points well enough to be certain, it would seem appropriate that patients who have not yet had surgery try to move as much lymph through the body as possible to see if the swelling subsides. For those who feel they are at risk for cancer, periodic lymphatic stimulation and/or drainage may be a healthy preventative measure. Once, however, the nodes are gone, it is more uphill.

One very simple but probably only minimally helpful exercise is to hold the affected (swollen) area upwards to see if gravity will permit some of the lymph to descend into areas that still have adequate drainage.

“To battle her son’s autism, Kazuko Curtin did more than look into a treatment — she started a clinic for it.
While the reasons remain unclear, hyperbaric chambers may help autistic children.

Twelve years ago, Curtin was told by doctors that her son had autism. In subsequent years, while attending conferences, she heard about treatment in a hyperbaric chamber, where pressure is increased in an attempt to boost the amount of oxygen in the child’s brain.

Curtin bought a machine, and today a hyperbaric chamber is one of the treatments offered by the CARE Clinics in Austin, Texas, and Tampa, Fla., which she opened last year.

“”Hyperbaric is very useful,”” said Curtin. “”You never think autistic children are going to stay inside the hyperbaric for 90 minutes, because they are very restless. What’s amazing — they like it! For some reason, they are very calm inside.””

Curtin is the not the first to use hyperbaric therapy, a procedure with little scientific backing for the treatment of autism. But a new, small study of 56 children treated at several small clinics may change that if the findings can be replicated.

Multiple Chemical Sensitivity Treatment

Public Health Issues in Medical Evaluation and Care of MCS Patients

Many physicians are uncertain how to approach the evaluation and care of persons who have multiple symptoms that attribute to low-level chemical exposure. Although medical approaches and therapies differ considerably because of differing beliefs about MCS by physicians, all individuals who report suffering from chemical sensitivities should receive a competent, complete medical evaluation and compassionate, understanding care. The goal of this care should be to promote health without causing additional harm. Individuals should not be subjected to ineffective, costly, or potentially dangerous treatments. Appropriate care for well-characterized medical and psychological illnesses should not be withheld or delayed. The ramifications of recommending functional changes in workplace or home settings should be carefully considered.

Medical Evaluation

The identification of MCS is based largely on the patient’s description of the symptoms and the relationship of these symptoms to environmental exposures. The evaluation of an individual for MCS should, therefore, begin a complete and detailed history, including a comprehensive exposure history. The ATSDR/NIOSH Case Study in Environmental Medicine—Taking An Exposure History (ATSDR, 1992) is a useful guide for physicians unfamiliar with taking an environmental exposure history.
